ты перезапускал клиент после изменения имени итема в патче ?

а что за сборка ?

В базе случайно нету таблицы ТВТ ?

Либо в базе etcitem, либо в патче itemname смотри там

А порт 3306 открыт ? именно порт самого мускула

Отключи файрвол или добавь в исключения порты
